A fatal industrial accident at ATI Ladish in Cudahy on Thursday, November 3, resulted in two casualties—only one of them resulted in death.

Officials say the incident occurred around 1:15 p.m. at the company’s 5481 S. Packard Avenue location, which is near Ladish Avenue.

The victim who died was a 49-year-old, later identified as Shawn Day, whose suspected death was trauma from being hit with a piece of metal in the head. He was set to have an autopsy the following day.

The second victim, who was only injured, was a 48-year-old from Cudahy who wasn’t identified. He is expected to live from his less severe injuries.

The Milwaukee County District Attorney’s Office, OSHA, and Cudahy police are cooperating in the investigation of the man’s death.
