A wrongful death lawsuit was filed on Monday, August 29, by the family of a Marine veteran who died from a toxic mix of drugs administered to him at a U.S. Veterans Affairs facility in Tomah, Wisconsin.
The federal lawsuit alleges that VA caregivers gave an improper prescription and dispensing of medicine to Jason Simcakoski, who died in 2014. It further alleges the VA failed to render necessary emergency care when he was found unresponsive. They also failed to come to a proper diagnosis regarding his mental health and substance abuse problems.
